Jesus Created Ministries (JCM) 2010 Creation Essay Contest Registration Form

By completing and signing this form, you agree to the following:

· The student is an Oklahoma resident between the ages of 12-18 on June 15, 2010.

· The student acknowledges that he/she agrees to the following 2 viewpoints:

1. The account of origins described in Genesis is a factual presentation of historical truth. All basic kinds of living things, including man, were made by direct creative acts of God during the creation week around 6,000 years ago. Subsequent variations of the original kinds have been bounded.

2. The Flood described in Genesis 6-8 was a real historical event, was global in its extent, and produced most of the fossil bearing strata present on the earth today.

· The submission shall be in accordance with the JCM Creation Essay Contest Guidelines.

· The essay must be the original work of the student without parental/guardian or other outside assistance, except for the following: help may be provided in determining appropriate reference materials for gaining knowledge on the chosen topic and for  properly referencing the material in the essay.  Note: properly footnoting reference material is important, see Reference Information Page at the link listed at the bottom of this registration form for more information.

· Submitted essays must not be published or distributed in any way until after winner announcements.

· The student and parent/legal guardian agrees to grant to JCM the nonexclusive right to use and publish their essay on the JCM website, and/or other printed publications.  The student may choose to only have their first name and hometown disclosed if full disclosure is not preferred.

· The general judging criteria is provided in the JCM Creation Essay Contest Guidelines.  The judges’ decisions are final and no explanations or details of results shall be required from judges or JCM.

· Payment for a winning essay will be made to the parent/legal guardian (based on the contact information below) on behalf of the student.  The contact information below will also be used to notify winners of their invitation to participate (with 3 other guest) in the Answers in Genesis Creation Museum trip planned for Aug 12-14, 2010.
